Registration Information

For continuity purposes (adding to or updating your information as your business grows), we recommend that a principal or key decision maker of your company complete the Supplier Registration Form.

Please Note: There may be sections of the form that DO NOT apply to you and your business.  Please skip those sections. Only “Required” fields are necessary to submit the form.

In order to expedite this process, you may want to assemble and do the following:

  1. Decide on your user name and password and write it down. Keep it secure.
  2. Principal owner information (for private companies only).
  3. Basic company information, including UNSPSC and NAICS.
  4. Average sales volume (this information is used to determine if a company is a small business according to the small business administration's requirements or if it is mid-sized, or large).
  5. Disadvantaged, Veteran, Minority, or Women-owned business Enterprise certification (if applicable).
  6. Certificate of Insurance (electronic version to upload).
  7. When you have completed the form, please click on the submit button and you will see a screen stating Supplier Registration Successful.
  8. If you need to revise or update your information, you can reenter the site with your user name and password.  You can update the fields that you need to; enter submit to save.
